Satellite Healthcare, Inc. and its affiliates and subsidiaries (“Satellite,” the “Company,” “we,” “us,” or “our”) is committed to protecting the privacy and security of the Personal Information we collect during the course of our business. This California Privacy Notice applies to our California employees (current and former) and their dependents, emergency contacts, beneficiaries, or authorized representatives (“Associated Individuals”) (collectively, “California Residents,” “you,” or “your”). PERSONAL INFORMATION WE COLLECT

“Personal Information” includes information that identifies, relates to, describes, is reasonably capable of being associated with, or could reasonably be linked, directly or indirectly, with California Residents but does not include information that is publicly available or information that is deidentified.

The following list identifies the categories of Personal Information Satellite may collect about you:

  • Identifiers, such as real name, alias, signature, postal address, unique personal identifier (as defined in the CCPA), online identifier, internet protocol address, email address, telephone number, account names, social security number, driver’s license number or state identification card number, vehicle information, passport number, familial associations, date of birth, or other similar identifiers.
  • Audio, electronic, visual, or similar information.
  • Biometric information.
  • Marital or domestic partner status; identifiers of spouses, domestic partners, dependents, emergency contacts and beneficiaries.
  • Medical conditions or information, including inoculation history; the presence of certain substances in your system as identified by urinalysis or other drug testing; identifying information related to your medical providers; insurance information, including, policy numbers.
  • Banking information, including bank account and routing numbers, credit card and other financial information.
  • Categories of protected classifications under California or federal law, including age, race, color, national origin, ancestry, citizenship, ethnicity, religion or creed, marital status, medical condition, physical or mental disability, genetic information, gender/sex (including gender identity, gender expression, sexual orientation, pregnancy or childbirth and related medical conditions), request for leave, and veteran or military status.
  • Internet or other electronic network activity information, including, but not limited to, browsing history, search history, cookies, geolocation data, and information regarding your interaction with an internet website, application, or advertisement.   • Professional or employment-related information, such as your employment history and professional licenses and identification numbers.
  • Educational information, including “education records” as that term is defined in the Family Educational Rights and Privacy Act (20 U.S.C. § 1232g; 34 C.F.R. Part 99)
  • Any Personal Information not covered above that you provide to Satellite or its service providers.
  • Inferences drawn from any of the information identified above to create a profile about you reflecting your preferences, characteristics, behaviors, attitudes, intelligence, abilities, and aptitudes.

If an employee gives us the Personal Information of another person, such as Associated Individuals, the employee must obtain that person’s permission to share their data and must share this California Privacy Notice with that person.

THE SOURCES FROM WHICH WE OBTAIN INFORMATION

We obtain Personal Information directly from you or indirectly from third-party sources, such as employees, employment or recruitment agencies and other referral sources, affiliates, business partners, contractors, service providers, and professional and educational institutions, among others.

THE PURPOSES FOR WHICH WE COLLECT YOUR PERSONAL INFORMATION

Satellite uses your Personal Information to verify your identity and credentials, to carry out background and reference checks in accordance with the law, for pre-employment and employment drug testing and medical screenings, to verify and monitor your work-related licenses, certificates, and trainings, and to comply with federal and state employment authorization and reporting requirements.

We also use your Personal Information for personnel, recruitment, management, operational and planning purposes. This includes managing compensation and expense reimbursements and administering benefits and health and wellness programs.

Satellite may also use your Personal Information in connection with investigations and audits; to research, analyze, evaluate, and maintain the health, safety, and efficiencies of our workforce, assets, and working environment; to fulfill the purposes for which you provided the Personal Information to Satellite; to comply with applicable legal or regulatory requirements; and to defend against any claims or threatened legal claims against Satellite or otherwise protect our legal interests. The Personal Information we collect may also be used in connection with a corporate transaction, such as a merger, sale, reorganization, or other transfer of business assets.

If Satellite uses your Personal Information to fulfill or meet some other purpose not explicitly provided herein, Satellite will describe the purpose at the point of collection.

THIRD PARTIES WITH WHOM WE DISCLOSE/SHARE YOUR PERSONAL INFORMATION

Satellite does not sell any of your Personal Information. We may disclose your Personal Information to third parties for business purposes. We may disclose your Personal Information with the following categories of third parties:

  • Affiliates or business associates.
  • Contractors and service providers.
  • Third parties with whom you direct us to share your Personal Information.
  • Third parties as required by law or legal process.
  • Federal, state, and local governmental agencies.
  • Legal counsel and accountants.
  • Educational, professional and health institutions to verify educational, employment and training background.

If you interact with our website, the Company uses your cookie data to help us compile aggregate data about website traffic and website interaction so that we can offer better website experiences and tools in the future. We or our service providers may share your cookie data with third party advertising and marketing partners for cross context behavioral advertising. When you first visit our website, you will be asked to “Accept All” cookies or “Reject All” cookies which are not classified as strictly necessary to the proper functioning of our website. DATA RETENTION

We keep your personal information for as long as needed or permitted in light of the purpose(s) for which it was obtained. In determining the retention period, we consider the length of our ongoing relationship with you. We also consider the laws to which we are subject and any retention period that may be advisable in light of our legal position (such as in regard to applicable statutes of limitations, litigation, or regulatory investigations).

YOUR RIGHTS WITH REGARD TO YOUR DATA

You have certain privacy rights with regard to your Personal Information:

  • Right to Know: ou may request that Satellite disclose:
    • Categories of personal information we have collected about you.
    • Categories of sources from which the personal information is collected.
    • The business or commercial purpose for collecting, selling, or sharing personal information.
    • Categories of any third parties to whom we disclose personal information.
    • The specific pieces of personal information we have collected about you.
  • Right to Change: You have the right to request that we correct any of your Personal Information in our possession that you believe to be inaccurate.
  • Right to Delete: You have the right to request that we delete Personal Information collected from you, subject to certain exceptions.
  • Right to Opt-out of Sale or Sharing: You have the right to opt-out of the sale or sharing of your Personal Information with third parties. Satellite does not sell any of your Personal Information. If you interact with our website, we share your cookie data with third-party advertising and marketing partners for cross context behavioral advertising. We recognize Global Privacy Control (GPC) signals, and if your web browser utilizes GPC, your opt-out preferences over the use of your cookie data will be honored. You may opt-out of our use of cookies for purposes other than as strictly necessary for the proper functioning of our website by selecting the “Reject All” option when you first visit our website.   • Right not to Receive Discriminatory Treatment: We will not discriminate against you for exercising any of your privacy rights.

HOW TO EXERCISE YOUR RIGHTS

You, or a person that you authorize to act on your behalf, may make a verifiable consumer request related to your Personal Information. All requests to opt-out of sharing or for information, deletions, or corrections must be made to:

Upon submitting a request, Satellite will attempt to verify your identity by matching identifying information provided by you (or your authorized representative) to the personal information maintained by Satellite. If a request is made by an authorized agent, Satellite will require you or your authorized agent to provide proof that you gave the agent permission to submit the request. The type of information needed to verify your identity will depend on the type of request you make. If we cannot verify your identity from the information already maintained by us, we may request additional information from you. We will never require you or your authorized agent to pay a fee for the verification of your request.

We will confirm receipt of your request within 10 days. You may make a request for access or data portability no more than twice within a 12-month period.

We will respond to your request no later than 45 calendar days after receipt of the request. The 45-day period will begin on the day that we receive the request. If we cannot verify your identity or the authorization for an agent to make a request on your behalf within the 45-day time period, we may deny the request. If necessary, we may take up to an additional 45 calendar days to respond to your request, for a maximum total of 90 calendar days from the day the request is received. If additional time is needed, we will notify you prior to the expiration of the initial 45-day period.
